After you submit this form you will get an invite from Box.com to a secure online folder to upload documents.

Submitting this form constitutes your permission for us to order a credit report.

If you use a "credit watch" service to monitor your credit activity don't be alarmed if you see a credit inquiry by "Settlement One", "Credit Plus" or "United One" which are the credit bureaus we use.

The process is usually completed 1 business day after you submit any supporting documents.

You may voluntarily send in a recent pay stub, tax returns from the last two years, W2's from the last two years and recent bank statements.
